Taylor Swift drops new single ‘Patient Zero’ about escaping a toxic relationship

Taylor Swift has released her new single Patient Zero, a track that explores escaping a toxic relationship and offering support to another woman who may be experiencing the same situation.

The 36-year-old singer released the song on Friday, September 25, as the lead single from The Life of a Showgirl: The Encore, a surprise expanded edition of her 2025 album featuring four new tracks.

In Patient Zero, Swift appears to address a woman involved with a charismatic but damaging man. Rather than simply reflecting on her own past, the song's narrator tells the woman that she understands what she is going through because she experienced the relationship herself.

Swift describes the man as someone with a "devil on his shoulder under his halo", while using the idea of toxins taking hold to describe the effect of the relationship. She then offers herself as someone the woman can turn to, saying she has already been through the same experience.

The song later makes clear that Swift's narrator had the man before eventually walking away from the relationship. The lyrics centre on recognising harmful patterns, recovering from them and helping someone else who may now be caught in the same situation.

Swift has not publicly confirmed that Patient Zero is about a particular former relationship. However, the song has already prompted fan speculation about which of her past relationships may have inspired its storyline.

The release follows several days of clues from Swift. She changed the "o" letters in her Instagram bio to zeroes, writing, "And, baby, that's sh0w business f0r y0u," before announcing the single following a countdown on her website.

The singer also teased the Patient Zero music video on September 24, revealing that it stars Dakota Johnson and Colin Farrell. A 13-second preview aired on CBS Mornings, while the full video is scheduled to premiere during the MTV Video Music Awards on September 27.

Patient Zero is one of four songs featured on The Life of a Showgirl: The Encore. The expanded edition also includes Cleveland!, Pink Clouding and Babylon. Swift said the songs were written during a trip to Sweden with her longtime collaborators Max Martin and Shellback, who also worked on the original The Life of a Showgirl.

The new material comes less than a year after Swift released The Life of a Showgirl, which became a major commercial success. Patient Zero also marks her latest original release following I Knew It, I Knew You, her contribution to the Toy Story 5 soundtrack.

With its focus on toxic relationships, recovery and female solidarity, Patient Zero introduces a different perspective to Swift's latest run of new music while leaving the identity of the relationship at the centre of the song open to interpretation.
