A month ago the 23-year-old chose to sit outside Karachi Press Club and refuse food, to demand the return of Zahid Baloch, the chairperson of the Azad group of the Baloch Students Organisation (BSO).
The missing man was allegedly picked up by security forces on March 18 in Quetta.
For a month Johar and other BSO activists searched for their chairperson, but in vain.
With the hunt attracting little attention, the central committee of the BSO decided to launch a hunger strike "unto death" and Johar offered himself as the maiden volunteer.
"We just want that at least the world must know what is going on with us and how the state is suppressing our movement by kidnapping and killing our fellows," Johar said.
The BSO is one of the largest Baloch student movements.
Some accuse it of links to violent separatist groups who have waged a 10-year insurgency, with regular bomb and gun attacks on security forces and state symbols.
Johar has lost 21 kilogrammes of his body weight since beginning his protest and his frailty is obvious as he becomes breathless while talking.
Doctors say he is heading towards death.
"I am quite worried about his survival as he is so stubborn that he refuses to take even any medication," Birma Jesrani, a doctor who regularly examines Johar, told AFP.
"I am afraid he could encounter a sudden medical condition any time and I don't think he could last another month."
Johar endures heat, humidity and the constant prick of mosquito bites in a tent in the teeming port city, a small book on Latin American revolutionary Che Guevara and a red-covered laptop by his side.
"I feel vertigo when I go to the toilet and walk even just a few steps. I just try to take 12 to 15 glasses of water daily," Johar told AFP.
"I cannot sleep and keep changing the side and posture till morning. I start reading books or messaging on social media but the headache doesn't let me do this for long," he said.
Despite the hardship, he says he is determined to carry on.
"We are fully committed to sit on hunger strike and if I die someone else will replace me," Johar said.
"We believe in peaceful struggle and we cannot pick up the gun, so this is our way to tell the world about our struggle."
In recent years many people suspected of links to Baloch separatist groups have mysteriously disappeared and never been seen again.
Sometimes they reappear after months or years in detention. Or dead, their bodies left by a road or in a riverbed, a strategy dubbed "kill and dump."
Baloch campaigners say the missing number in the thousands but the authorities dispute this, putting the figure at less than 100.
They also say that militant separatist groups are responsible for many of the disappearances.
"We are undeterred and we shall continue our struggle even if 'something happens' with our chairperson," Johar said, hinting at his worst fears about the missing student leader.

Use some common sense If his house was under "fire" then he was already burnt dead. He & his "backers" have enough bread n butter to put him on show in front of press club which is almost 700 Kms away from his academics, while his peers are receiving Fee Reimbursement Scheme (PMFRS) for Less Developed Areas (LDAs) yesterday in Quetta.
We have seen these dramas on & on, tell you what, in interior of Sindh when annual exams are about to commence the political goons (Comrades) put some puppets on hunger strike one reason or the other, just to get a free pass in exams.
Speaking about knowledge, I have more than enough knowledge about Balochistan cuz I have spent a good about of my life in Balochistan. I am one of those lucky persons who has a contribution in building Gwadar when it was just connected through Coastal Highway 2004.

Unfortunately Baloch's are also one of the suppressed nation in Pakistan. Punjabi ruling elites doesn't want to give equal rights to other nations live in this country. If the discrimination against small nation continues it will be hard for Pakistan to survive as a federation.
I hope the hype anna hazare managed to get with his call for hunger strike against terrorism, this guy also get that.. I see him on daily basis in the camp at press club as i worked there, but no media has yet highlighted their struggle, the way they deserve to be. We all know that without being a sensationalized news item, you can't get anybody's attention.

I know I shouldn't and by-now I should be used to it but I always get shocked to see the ignorance of people, all they care about it that Pakistan is better than India They don't give a fuss on Balochistan or the people there. I am terribly sorry but the Person on hunger strike ceases to be a student once he joined hunger strike on the pretext of a Political Student Organization with tainted history.
A student job is to study peacefully and avoid politics. After completion of your studies you can be whatever you like, be a political leader or pick a gun to further your cause. But whatever one chooses he has to choose wisely cuz their will be repercussions, nothing in life will be gifted in a plate for free.
Now, for the person sitting in a hunger strike, I have to say that either return to studies or die quickly, please.
And further, ET, you only got time for those fake hunger sitters, your reporter and web space does not have time to do an investigative piece that what is the situation of our academic institutions in Balochsitan, why there are no funds but for MPs, why there is no academic staff but regiments of MPs/MNAs security staff is floating on govt expenses etc.
