Pakistan seems to be losing ground in the battle against poliovirus. The recent confirmation of wild poliovirus type-1 in four children from Balochistan and Khyber-Pakhtunkhwa serves as a stark reminder of the ongoing challenges in the fight against polio in Pakistan. With a total of 37 cases reported nationwide this year, the urgency for comprehensive immunisation strategies is more critical than ever. The affected children, including a girl from Pishin and boys from Chaman and Noshki in Balochistan, highlight the geographical disparities in polio cases.

Alarmingly, Balochistan alone accounts for over half of the cases, revealing significant vulnerabilities in vaccination efforts in this region. Insecurity, community resistance, and local protests have disrupted immunisation campaigns, leaving many children unvaccinated and susceptible to the virus. The situation in Lakki Marwat marks the first detection of polio this year, illustrating that the virus is not just confined to historically high-risk areas. This indicates a broader risk, as pockets of unvaccinated children across various regions create an environment conducive to virus transmission. The reliance on staggered and postponed campaigns due to local unrest must be addressed, as every missed child represents a potential vector for further outbreaks.

As genetic sequencing of the recent cases is underway, it is essential for public health officials to communicate transparently about these findings and implement strategies that prioritise high-risk areas. Community engagement is crucial; building trust through dialogue and addressing concerns about vaccination can help dismantle resistance to immunisation efforts. The resurgence of polio in Pakistan underscores a pressing need for renewed commitment to vaccination programmes. It is imperative that all stakeholders, from government to local communities, unite to ensure that every child receives the protection needed against this preventable disease. The path to eradication requires vigilance, adaptability and unwavering dedication.
