LARKANA: A three-member inquiry committee, constituted by the Sindh health services director general, following media reports, held responsible on Tuesday the district health officer, Expanded Program on Immunisation (EPI) in-charge and storekeepers, for mismanagement and negligence because of their failure to utilise Hepatitis B and C vaccines on time.
